How to Create and Edit a Survey Form

A. Create a Survey Form (First Time)

Step 1: Open the Survey app from the sidebar.

Step 2: Select the survey configuration you want to work on.

Step 3: Click the Form tab.


If no form exists yet, you will be prompted to create one.

Step 4: Add your first question:

  • Choose a question type (text, textarea, date, choice, rating, upload, stars, faces, etc.)

  • Enter the question text

  • Add placeholders or options if required

Step 5: Add more questions by clicking Add question, selecting a type, and entering the question.

Step 6: Click Save when you are done.


B. Edit an Existing Survey Form

Step 1: Open Survey, select your survey configuration, and go to the Form tab.

Step 2: Click Edit to open the form editor.



C. Edit Questions

Step 1: Click a question to update:

  • Question text

  • Placeholder or default value

  • Required (toggle on or off)

  • Question type

Step 2: For choice or checkbox questions, manage options:

  • Add options

  • Remove options

  • Reorder options


D. Add More Questions While Editing

Step 1: Click Add question.


Step 2: Choose a question type and enter the question details.


Step 3: Enable Required if the question must be answered.



E. Save Your Changes

Step 1: Click Update to save your edits.
